Fioricet is a medication commonly prescribed for tension headaches. It contains a combination of acetaminophen, butalbital (a barbiturate), and caffeine. Due to its components, Fioricet can interact with a number of drugs, leading to serious side effects or reduced efficacy. Key interactions include:
- Alcohol: Combining Fioricet with alcohol can increase the risk of liver damage (due to acetaminophen) and enhance the sedative effects of butalbital.
- Opioids and Sedatives: Drugs like benzodiazepines (e.g., Xanax, Valium) and opioid pain medications (e.g., hydrocodone, oxycodone) can increase the risk of extreme drowsiness, respiratory depression, and overdose when taken with Fioricet.
- Blood Thinners (e.g., Warfarin): Acetaminophen can enhance the effects of blood-thinning medications, increasing the risk of bleeding.
- Antidepressants: Monoamine oxidase inhibitors (MAOIs) and certain SSRIs may interact with the caffeine in Fioricet, leading to increased heart rate, blood pressure issues, and other nervous system side effects.
- Other Barbiturates or CNS Depressants: Fioricet already contains butalbital, a central nervous system (CNS) depressant. Taking additional CNS depressants can amplify the risk of excessive sedation and respiratory depression.
- Medications Affecting Liver Enzymes: Drugs like rifampin, phenytoin, or carbamazepine can affect liver enzymes, potentially increasing the metabolism of Fioricet, reducing its effectiveness or increasing the risk of side effects.
- Stimulants: Caffeine in Fioricet can interact with other stimulants, leading to overstimulation and cardiovascular side effects like palpitations or increased blood pressure.
Using this medicine with any of the following medicines is not recommended. Your doctor may decide not to treat you with this medication or change some of the other medicines you take.

Other Medical Problems
The presence of other medical problems may affect the use of this medicine. Make sure you tell your doctor if you have any other medical problems, especially:
- Alcohol abuse, history of or
- Drug dependence, or history of or
- Weakened physical condition—Use with caution. May increase risk for more serious side effects.
- Kidney disease or
- Liver disease—Use with caution. The effects may be increased because of slower removal of the medicine from the body.
- Porphyria (an enzyme problem)—Should not be used in patients with this condition.
- Stomach problems—Use with caution. May make these conditions worse.
